Direct Deposit Authorization Form

Authorizes benefit payments to be deposited directly into the Trust United client account so bills can be paid on time.

Consent Forms

Permission to communicate with the Social Security Administration, healthcare providers, landlords, and other parties on the beneficiary's behalf.

Government-issued ID

Driver's license, state ID, or passport for the beneficiary.

Benefit Verification Letter

Current letter from SSA confirming the type and amount of benefits received.

Lease Agreement

Current signed lease or housing agreement so rent can be paid directly to the landlord.
